What is Avlimil?

Clinical Summary

Avlimil is a dietary supplement that is being promoted to help alleviate symptoms associated with female sexual dysfunction. It consists of sage leaf, red raspberry leaf, kudzu root extract, red clover extract, bayberry fruit, damiana leaf, valerian root, and ginger root. Sexual dysfunction may involve loss of sexual desire, difficulty in maintaining arousal, painful intercourse, or difficulty in achieving orgasm. It may result from factors such as stress, hormonal imbalances, cancer treatment involving surgery, radiation, chemotherapy or hormone therapy, use of antidepressants, or menopause. Avlimil is thought to enhance sexual satisfaction by increasing blood flow and by promoting muscle relaxation. Some of the components may improve circulation.
Results from in vitro and in vivo assays showed that Avlimil exhibits both estrogenic (at low concentration) and antiestrogenic (at higher concentrations) effects in a dose dependent manner. A small unpublished clinical trial involving 49 women found Avlimil to have a positive effect on sexual response. However, the ingredients in the commercial product are different from those used in the study.
Avlimil has not been studied in cancer patients. Since this product contains botanical ingredients that have estrogenic effects, it is not appropriate for patients with hormone sensitive cancers.

What is Agaricus Mushroom?

Clinical Summary

Agaricus blazei is an edible mushroom native to Brazil and cultivated in Japan for its medicinal uses. It has been used to treat arteriosclerosis, hepatitis, hyperlipidemia, diabetes, dermatitis, and cancer. In vitro experiments and studies done in mice have shown that Agaricus has immunomodulatory, antitumor, and antimutagenic properties. The polysaccharides and anti-angiogenic compounds present in Agaricus are thought to be responsible for its antitumor properties. Such effects are believed to be exerted by immunopotentiation or direct inhibition of angiogenesis. A recent randomized study showed that oral administration of Agaricus extract improved the natural killer cell activity and quality of life in gynecological cancer patients undergoing chemotherapy. However, more studies are needed to confirm these observations. Agaricus was also shown to have antidiabetic effects in vitro and in animal studies. In addition, results from a study done in human subjects with type 2 diabetes suggest benefits of Agaricus extract in improving insulin resistance, and a pilot study indicates that Agaricus extract may reduce weight, body fat, and serum glucose and cholesterol levels in healthy individuals.

While a small pilot study reported that Agaricus extract may improve liver function in patients with hepatitis B, liver damage and deaths along with cheilitis following consumption of Agaricus have been reported. Due to its immunopotentiating effects, Agaricus can potentially interfere with immunomodulating drugs although such interactions have not been studied.

Agaricus is an edible fungus. It is available as freeze-dried mushroom or as concentrated liquid extracts, teas, or capsules. The whole mushroom is often added to soups, sauces, or hot teas.

From the Sanskrit words “ayur” (life) and “veda” (knowledge), Ayurveda originated in India over 3000 years ago. Treatment is patient-tailored and consists of oral formulations containing herbal, mineral, spice, and animal components, mental balance through yoga or meditation, purgatives, and dietary and lifestyle changes. It is available in at least 10 clinics in North America, but there is no licensure for practicing Ayurveda in the United States. Ayurvedic medicinals, which can contain from a few to hundreds of components, are commonly used to treat chronic diseases such as diabetes, rheumatoid arthritis, Parkinson's disease, acne, obesity, and cancer. Many of the more frequently used herbs have been studied extensively in vitro and in vivo and show antioxidant, antitumor, antimicrobial, immunomodulatory, hypoglycemic, or antiinflammatory properties. Clinical trials show benefit of various formulations in treating acne, rheumatoid arthritis, fibromyalgia, diabetes, hyperlipidemia, anemia, hepatitis, obesity, dyspepsia, and constipation. The effects of meditation in reducing anxiety, lowering blood pressure, and enhancing general well-being have also been confirmed. The World Bank is funding research in India to evaluate ayurvedic medicines for anemia, edema during pregnancy, and postpartum complications. There is no documented value for Ayurvedic therapies in treating cancer. However, certain modalities can be used to relieve cancer related symptoms and to improve quality of life. Recent studies demonstrate that yoga helps improve sleep pattern in lymphoma patients and also reduces symptoms of stress in breast and prostate cancer patients.Little is known about the drug interactions between Ayurvedic and modern drugs. Numerous cases of lead poisoning from Ayurvedic preparations have been reported.

Clinical Summary

Scutellaria barbata is a perennial herb prevalent in Korea and southern China. It is used along with other herbs in traditional Chinese medicine to treat bacterial infections, hepatitis, tumors, and as a diuretic.

Data from in vitro studies suggest that S. barbata has antibacterial, antimutagenic, and anticancer properties. The flavonoid components are thought responsible for the anticancer activity. Small studies of S. barbata show that it is safe and may benefit patients with advanced breast cancer. Further research is needed.

Clinical Summary

Bilberry is derived from the fruit of the tree. It is available as a dietary supplement to help improve eyesight. Several human studies suggest that bilberry anthocyanosides prevent diabetic retinopathy and improve visual acuity and retinal function. One human study also suggested that bilberry extract may significantly reduce the proliferation of colorectal cancer tumor tissue. In vitro studies have also indicated that bilberry may have anticancer activities. Coumarins present in bilberry may interact with platelets and have an additive effect with blood thinners. Bilberry fruit should not be confused with bilberry leaf, which may have hypoglycemic effect.

Clinical Summary

Bitter melon is a perennial plant that grows in the tropical and subtropical regions of Asia, Sourth America, East Africa, and the Carribean. It is used both as food and in medicine to treat diabetes, cancer, viral infections, and immune disorders. The nutritional value of bitter melon is due to its high mineral and vitamin content, whereas its bitterness is attributed to the presence of alkaloids, momordicosides, and momordicines.
In vitro and animal studies indicate anticancer, antiviral, and lipid lowering effects. Bitter melon was also shown to exert significant hypoglycemic effects in both healthy and diabetic patients, but further studies are required to recommend its use.
Bitter melon had no effect on natural killer cell activity in a study of cervical cancer patients.

There is a potential for additive effects when bitter melon is combined with insulin or oral hypoglycemic agents.

Clinical Summary

Bladder wrack is a seaweed prevalent on the Atlantic and Pacific coasts from Europe to Asia. It is often referred to as brown kelp but it should not be confused with “kelp,” another species of seaweed. Bladder wrack is consumed as food and medicine and is a rich source of iodine. It is used in traditional medicine to treat hypothyroidism due to iodine deficiency and has been proposed as a weight loss supplement. No clinical studies have verified this effect. Bladderwrack is believed responsible for the reduced risk of estrogen-related cancers in Asian populations and may improve menstrual symptoms. Further studies are needed to clarify such effects.
Bladder wrack extract also demonstrated chemopreventive, anti-collagenase and antioxidant properties; topical application of bladder wrack extract may help improve skin.
Bladder wrack should be used with caution in patients with hormonal-sensitive cancers.
